This application is for current students who are attending USAO during the 2019-2020 year and plan to continue their studies during the fall 2020 and spring 2021 term(s) at USAO. It includes the optional opportunity to apply for 2020-2021 USAO Foundation Scholarships for continuing students. All students should be sure they have a valid, current mailing address on file with the USAO Registrar.

The FAFSA is required to apply for all types of federal financial aid; including grants, loans, and work-study. Early application is key to maximize funding opportunities. Currently, Oklahoma Tuition Aid Grant (OTAG) deadline and funding availability for 2019-2020 is unknown. You are encouraged to submit your 2019-2020 FAFSA by December 1, 2018 to receive best consideration for OTAG funding for the 2019-2020 year; however, the 2019-2020 FAFSA may be submitted anytime from October 1, 2018 through June 30, 2020. Submit your 2019-2020 FAFSA as early as possible at or through the my StudentAid Mobile App. 
Will you receive any scholarships for 2019-2020 from clubs, organizations, etc. outside of USAO? (Other than the prior sources; do not include Oklahoma's Promise (OHLAP), USAO scholarships, or federal grants such as Pell.)*
The various USAO Foundation Scholarships are merit awards based primarily on scholastic promise and achievement; although character, service and leadership potential are also considered. Generally, scholarships are awarded to full-time students who maintain a minimum 3.00 GPA without consideration of need for financial assistance. Priority will be given to students who pre-enroll for fall 2019 and whose financial aid packages are complete. Applications received after April 1, 2019 will be subject to availability of funds. Recipients are required to correspond with the donors of the scholarship fund at the address provided on the award letter.

By submitting this form, you certify that the information you have provided is true and correct to the best of your knowledge. You authorize the release of GPA information by USAO for the purpose of obtaining scholarship/grant(s). You also authorize the disbursement of Federal Title IV funds to pay any previous term student account balance of not more than $200 (limit under federal law). You understand that you have the right to cancel any student loan up to 14 days after its disbursement. You also have read USAO's Satisfactory Academic Progress (SAP) Policy and acknowledge that SAP is required to receive financial aid. You understand that e-mail correspondence regarding USAO financial aid will be sent to your student e-mail address only and that you are responsible for checking your student e-mail account regularly.
