2016-2017 Financial Aid Office Update Form

This application is for current students continuing their studies during the fall 2016 and spring 2017 term(s) at USAO.  It includes the optional opportunity to apply for USAO Foundation Scholarships for continuing students.

If yes, you are required to report below those colleges and universities attended and the dates of attendance. Complete academic transcripts must be submitted to the Registrar and Financial Aid Offices before aid will be considered.

The FAFSA is required to apply for all types of federal financial aid; including grants, loans, and work-study. File your FAFSA by March 1, 2016 to meet the Oklahoma Tuition Aid Grant Program deadline for the 2016-2017 year.

If yes, federal policy requires ALL outside aid sources to be reported to the Financial Aid Office. Please provide the organization or name of the scholarship and the amount you expect to receive for 2016-2017.

The various USAO Foundation Scholarships are merit awards based primarily on scholastic promise and achievement; although character, service and leadership potential are also considered. Generally, scholarships are awarded to full-time students who maintain a minimum 3.00 GPA without consideration of need for financial assistance. Priority will be given to students who pre-enroll for fall 2016 and whose financial aid packages are complete. Applications received after April 1, 2016 will be subject to availability of funds. Recipients are required to correspond with the donors of the scholarship fund at the address provided on the award letter.
By submitting this form, you certify that the information you have provided is true and correct to the best of your knowledge. You authorize the release of GPA information by USAO for the purpose of obtaining scholarship/grant(s). You also authorize the disbursement of Federal Title IV funds to pay any previous term student account balance of not more than $200 (limit under federal law). You understand that you have the right to cancel any student loan up to 14 days after its disbursement. You also have read USAO's Satisfactory Academic Progress (SAP) Policy and acknowledge that SAP is required to receive financial aid. You understand that e-mail correspondence regarding USAO financial aid will be sent to your student e-mail address only and that you are responsible for checking your student e-mail account regularly.
