Vehicle Request Form
Our vehicle request form is available online. Once submitted, the form will be sent to both the maintenance office and your supervisor for approval.

*People Mover (21/24 seats)
*People Mover w/ chair lift (20 seats)
*must have valid CDL to operate people movers
Vehicle Pick-Up and Drop-Off Instructions
Pick-Up
Your vehicle number assignment will be posted on the office board for reference.
If you are picking up your vehicle during office hours:
- Check the board for your assigned vehicle number
- Pick up your keys from inside the office
If you are picking up after hours:
- Check the maintenance office white board for your assigned vehicle number.
- Use the key box located on the office exterior to access your vehicle’s key.
Drop-Off:
Upon returning the vehicle, please ensure it is parked in the designated area.
Return the key to the key box or the key drop located on the car barn door.
Please report any physical or mechanical damage on the FMX maintenance ticketing system, Linked here: GO FMX portal
If you have reserved a vehicle and do not already have the key box access code, please reach out to dmcallister@usao.edu or USAO Security (405-222-8066) and you will be issued the code.
Roadside Assistance
We have partnered with Enterprise for our fleet management services.
As part of this collaboration, roadside assistance will now be provided through Enterprise on all of our travel vehicles.
Should you require roadside assistance, simply call 800-325-8838, option 4, and provide the last four digits of the vehicle’s VIN. The roadside assistance number is also posted on the driver's side sun visor for your convenience.
Vehicle VIN Locations

The VIN (vehicle identification number) can be found on the driver’s side dashboard where the windshield glass meets the hood (see Illustration: Front View) or by the driver’s side door jamb (see Illustration: Top View)
Fuel Cards
Each travel vehicle now has its own WEX gas card located in the vehicle’s glove box.
Please use this card exclusively for fuel purchases for that vehicle.
Do not use University p-cards for fuel.
Driver ID’s have been sent out for each regularly travelling department. If you do not travel regularly and will be needing to purchase fuel on your trip, please reach out to Danielle McAllister at dmcallister@usao.edu to obtain your Driver ID.
If you have any Fuel Cards please return them to the maintenance office.
