INCIDENT REPORTING

Thank you for taking the time to report concerning behavior. Your concerns may include, but are not limited to, potential violations of the Code of Student Conduct; complaints; concerns related to possible violations of USAO’ Sexual Harassment, Sexual Assault, Sexual Misconduct and Title IX Policy such as gender based discrimination, dating violence, domestic violence, hostile work environment, sexual assault, nonconsensual sexual contact, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct; and general behavioral concerns regardless of whether they occur on campus or off. Please note that submitting this report is not considered filing a complaint. 

Upon receipt of this report, the appropriate USAO personnel will review the matter and take proper steps to investigate, stop, prevent, and remedy prohibited conduct in accordance with USAO policies.

Important note: This form may not be reviewed after-hours, on the weekends, or during university holidays or breaks. If you need to report an emergency, call USAO Security at (405) 222-8066, or dial 911.

This tool is intended to address concerns in a safe, timely, and supportive manner, with the goal of connecting individuals to resources they need.

If you have any questions regarding filing a report, please contact equalopportunity@usao.edu. Anonymous reports will be accepted but may not be investigated. Lack of information will delay processing of referrals. It is extremely important that you include complete and detailed information. The filing of an incident report does not constitute filing an official police report. The University strives to uphold your privacy. It is important to note that some information may be required to be shared with appropriate staff members, law enforcement, or other agencies so all reports may not be confidential.
