Office of Accessible Education Forms

All students requesting support must fill out the proper forms. Eligibility is then based on qualifying disability documentation and assessment of individual need. This is done on a case-by-case basis. To begin the process, please fill out the intake form from below and provide the appropriate documentation.

Along with filling out the forms and providing the appropriate documentation, an intake must be done with the Office of Accessible Education. If approved to receive services, an accommodation letter will be written to each of your professors detailing what accommodations are recommended. The student is then responsible for picking up the letters from the Office of Accessible Education, distributing them to the appropriate professor, and discussing the recommended accommodations. When a student enrolls for a new semester it is required that they provide their schedule to the Office of Accessible Education and request new letters for the upcoming semester.

CONFIDENTIALITY

The Office of Accessible Education will maintain the confidentiality of these diagnostic reports to the extent permitted by law and will not release any documentation without a student’s informed written consent, except as required by law or as deemed necessary to prevent harm to the student or others.

DEFINITIONS

Major life activity: Walking, sitting, standing, lifting, reaching, seeing, hearing, speaking, breathing, learning, working, caring for one’s self, and similar activities

Reasonable and appropriate accommodation: Change or modification that enables a student with a disability to enjoy equal opportunity and/or access to university facilities, programs, and activities, provided fundamental alteration would not result from the modification.

Reasonable accommodation is required for students with known disabilities. USAO is not required to provide “best” or “most desired” accommodations but rather a reasonable accommodation sufficient to meet accessibility needs. Department director or higher-level administrative staff must prepare a written explanation and consult with the ADA coordinator before denying a student’s request for accommodation.
